What are the responsibilities and job description for the Care Manager position at State of Louisiana?
The CONTRACTOR shall serve as the Care Manager at R.I.S.E. Treatment Center for Northeast Delta Human Services Authority (NEDHSA). The CONTRACTOR serves as a central coordination point for integrated care delivery to pregnant/parenting women with substance use disorders and co-occurring conditions and their dependent children. The Care Manager ensures continuity of care, reduces barriers to treatment, strengthens community integration, and supports stabilization, recovery, parenting capacity, and long-term wellness.
The Care Manager functions as an advocate, systems navigator, and collaborative team member within the R.I.S.E. interdisciplinary care model. The incumbent will complete NEDHSA employee orientation/training as scheduled.
OUTCOME(S):
CONTRACTOR core responsibilities include:
- Serve as primary care coordination contact for assigned residents.
- Advocate for high-risk and at-risk clients to promote recovery and stabilization.
- Establish a therapeutic, strengths-based working relationship.
- Provide culturally responsive, trauma-informed services.
- Conduct comprehensive Care Management Assessment immediately following intake.
- Identify social determinants of health impacting treatment outcomes.
- Develop and update individualized service coordination plans monthly.
- Utilize designated referral forms and document all referrals in the EMR.
- Coordinate referrals to obstetric, primary care, MAT, pediatric, housing, education, employment, and community services.
- Follow up to ensure services were accessed and barriers addressed.
- Assist residents in developing community living skills.
- Support enrollment in vocational, educational, and parenting programs.
- Coordinate discharge planning beginning at admission.
- Make collateral contacts with authorized family members and supports.
- Coordinate with DCFS, courts, and child welfare entities as needed.
- Support parenting stabilization and family reunification efforts.
CRITICAL INCIDENT REPORTING
CONTRACTOR is responsible for following the NEDHSA Critical Incident Reporting Policy and ensuring NEDHSA receives all Critical Incident Reporting Policy and ensuring NEDHSA receives all critical incidents reports in accordance with the guidelines set forth in the policy.
- High School Diploma or GED required
- Minimum of 1 year experience in behavioral health, inpatient and/or residential care
- Must pass background check and drug screening
- Valid driver’s license and the ability to lift up to 25 lbs
- Ability to maintain confidentiality and adhere to ethical standards in all interactions with clients and staff
CONTRACTOR shall perform services as set forth below:
- Serve as care manager and shall act as an advocate for high-risk/ at-risk clients to aid in the recovery and stabilization of those clients served.
- Meet with clients regularly based on established level of need to coordinate services and continue with service planning with other agencies and providers involved with the individual.
- Enhance treatment services provision, community integration by contacting other entities to arrange community access and involvement, including opportunities to learn community living skills, and use vocational and recreational services.
- Make collateral contacts with significant others and assists the person in identifying existing supports.
- Monitor progress and service delivery.
- Provide education, supportive counseling, and psychoeducation which guides the client and develops a supportive relationship that promotes the treatment services being provided.
