Summary
The Social Services Care Coordinator (Licensed Social Worker, LSW) supports Maine residents with disabilities who present complex healthcare, financial, and placement needs. Working within the Complex Case Unit (CCU), this role coordinates interdisciplinary providers and community resources to achieve safe discharge planning and service access in the least restrictive setting. The position requires strong care coordination, case management, interagency collaboration, and documentation skills.
Core Responsibilities
- Coordinate complex care management and discharge planning for members with disabilities, aligning providers, community partners, and family/supports.
- Facilitate team meetings for members lacking discharge plans until safe placements or resolutions are secured.
- Document team notes, interventions, and follow‑ups in CCU systems per protocol; maintain accurate, timely case records.
- Consult with members and teams to navigate services/resources (e.g., housing, behavioral health, LTSS, transportation), ensuring access to a least restrictive environment.
- Partner with providers to develop individualized plans of care with measurable goals and timelines.
- Build and maintain collaborative relationships with hospitals, community agencies, and service providers to improve outcomes and reduce barriers.
- Assess member eligibility for programs/waivers (e.g., BIW, DD, APS, other Medicaid/LTSS programs).
- Perform additional duties as assigned by the Department.
Qualifications
Minimum Requirements
- Six (6) years of education and/or professional experience in social services casework, social services program development/delivery
- Active Licensed Social Worker (LSW) issued by the Maine State Board of Social Worker Licensure.
Knowledge, Skills, and Abilities
- Knowledge of standards of care for health care facilities, agencies, and practitioners; Department programs, policies, procedures, and guidelines.
- Knowledge of client rights, confidentiality, and advocacy.
- Knowledge of community resources and service systems across health and human services.
- Strong written and verbal communication; ability to present and document clearly.
- Ability to collect and analyze casework material, formulate recommendations, and make independent decisions.
- Ability to maintain objectivity, professionalism, and effective working relationships across interdisciplinary teams.
- Proficiency with Microsoft Office; familiarity with care coordination platforms/EHRs is a plus.
Schedule
- Full‑time (40 hours/week), Monday–Friday (flexibility around hours within reason)
- Hybrid Work Schedule - Remote 3 days per week and Onsite 2 days per week
