The Community Hospice has an exciting opportunity for a Per Diem Registered Nurse to join our dedicated Hospice Home Care Team serving patients and families throughout Schenectady and Albany Counties.
Availabilityneededfor this per diem position would beduring the week (Mon-Fri).Hours generally needed are 8a-4:30p. No weekends or holidays

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

What you will do:
- As a Hospice Home Care RN, you will provide compassionate care and support to patients and their loved ones in their homes while working collaboratively with an experienced interdisciplinary team.
- Help coordinate and implement individualized plans of care that honor each patient's goals, comfort, and quality of life.
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
- Valid Driver's License and Reliable vehicle
- Passion for providing patient-centered, compassionate care
- Strong communication and teamwork skills
