Our Center of Rehab offers specialized subacute rehabilitation services in a supportive, collaborative environment. We are seeking a skilled Clinical Rehab Coordinator to join our leadership team and help ensure outstanding clinical care for our rehab patients.
This position is available due to the planned retirement of our current RN Manager in December. We are hiring now to ensure the selected candidate has the benefit of comprehensive onboarding and training alongside the outgoing coordinator, allowing for a smooth and supportive transition into the role.
The Clinical Rehab Coordinator is responsible for daily clinical oversight and coordination within the rehab unit. This role serves as a mentor and resource for nursing staff, ensuring safe, high-quality patient care while supporting seamless admissions, discharges, and care planning.
Key Responsibilities
- Daily Clinical Oversight
- Review and synthesize clinical information from 24-hour reports and nursing staff.
- Present daily clinical updates during stand-up meetings.
- Follow up on clinical concerns, delegating appropriately and ensuring full resolution.
- Provide daily updates to the Clinical Rehab Manager.
- Nursing Staff Support & Accountability
- Conduct regular rounding to monitor performance and assist staff as needed.
- Hold nursing staff accountable to expectations through supportive coaching.
- Provide mentoring, education, and corrective feedback to promote continuous improvement.
- Patient Admissions & Discharges
- Coordinate seamless patient move-ins, including interdisciplinary planning for multiple admissions.
- Ensure completion of admission assessments and proper documentation.
- Lead interdisciplinary team meetings for discharge planning, including education and continuity of care at discharge.
- Patient Care Coordination
- Participate actively in care conferences and interdisciplinary care planning.
- Conduct virtual visits as needed to maintain continuity of care.
- Provide clinical expertise for complex medical concerns and patient condition changes.
- Education & Mentorship
- Support the professional growth of nurses and CNAs through daily coaching.
- Reinforce best practices in clinical assessment, documentation, and patient care.
- Identify staff training needs and provide targeted support.
- Regulatory & Quality Compliance
- Ensure compliance with state, federal, and organizational standards of care.
- Participate in quality improvement reviews, audits, and documentation checks.
- Collaborate with leadership on policies, procedures, and regulatory readiness.
Qualifications
- Current Wisconsin RN license in good standing.
- Minimum 2 years of RN experience in skilled nursing, subacute rehab, or long-term care.
- Strong clinical assessment, communication, and organizational skills.
- Leadership experience or demonstrated leadership ability preferred.
- Proficiency with electronic medical records (PointClickCare preferred).
- Experience with Medicare, MDS, and interdisciplinary care planning desirable.
