We are seeking a Registered Nurse to provide care to our hospice patients as an RN case manager to our growing hospice in our Sheboygan and Ozaukee counties.
The Hospice Care Manager is responsible for managing and coordinating the delivery of hospice care services to patients and their families. This role involves working closely with interdisciplinary teams, healthcare providers, and community resources to ensure that patients receive comprehensive and compassionate end-of-life care. The Hospice Care Coordinator serves as the primary point of contact for patients and families, providing support, guidance, and information throughout the hospice care journey.
