Work Schedule:

Full-time position, weekends on and variable shifts during the week. Day shift with the exception of swing shifts as needed.

Job Summary

The Meds-to-Beds / Transitions of Care Pharmacist at St. Luke’s Health System – Nampa is responsible for facilitating safe, efficient, and patient-centered medication transitions from hospital to home. This role focuses on bedside medication delivery, discharge medication reconciliation, patient education, and coordination with outpatient pharmacy services to reduce readmissions and improve medication adherence.

The pharmacist collaborates closely with inpatient care teams, case management, nursing, and outpatient pharmacy staff to ensure patients receive their medications prior to discharge and understand their therapy plan.

Key Responsibilities

Transitions of Care & Patient-Centered Services

  • Perform comprehensive medication reconciliation at discharge, identifying and resolving discrepancies.
  • Coordinate bedside delivery of discharge medications (“Meds to Beds”) in collaboration with outpatient pharmacy services.
  • Provide patient counseling and education on new and existing medications, including dosing, administration, side effects, and adherence strategies.
  • Assess barriers to medication access (e.g., cost, insurance, transportation) and coordinate solutions.
  • Ensure timely access to medications prior to discharge to prevent therapy gaps.
  • Facilitate prior authorizations, medication access programs, and insurance coordination as needed.

Clinical and Patient Care

  • Review and verify discharge medication regimens for appropriateness, including dosing, interactions, contraindications, and duplications.
  • Provide recommendations to optimize therapy and ensure alignment with evidence-based guidelines.
  • Collaborate with physicians, nurses, and case managers to resolve medication-related issues prior to discharge.
  • Support high-risk patient populations (e.g., anticoagulation, heart failure, oncology, complex polypharmacy).
  • Document interventions, education, and outcomes in the electronic medical record.

Medication Preparation and Distribution

  • Coordinate with outpatient pharmacy teams to ensure accurate and timely dispensing of discharge medications.
  • Ensure compliance with all federal and state regulations, including controlled substance handling.
  • Verify prescriptions and facilitate efficient workflow between inpatient and outpatient pharmacy operations.

Supervision and Collaboration

  • Work collaboratively with pharmacy technicians and outpatient pharmacy staff to ensure efficient Meds-to-Beds workflow.
  • Provide guidance and oversight to support staff involved in medication delivery and coordination.
  • Serve as a resource for nursing and care teams regarding discharge medication processes.

Operational and Administrative Duties

  • Maintain accurate documentation related to medication reconciliation, patient counseling, and interventions.
  • Support compliance with regulatory and accreditation standards (e.g., Joint Commission, state board of pharmacy).
  • Participate in quality improvement initiatives focused on reducing readmissions and improving medication adherence.
  • Assist with program development and optimization of transitions of care services.

Education

  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree from an accredited college or university of pharmacy.

Experience

  • 0 years of experience required.
  • Experience in transitions of care, ambulatory care, or outpatient pharmacy preferred but not required.

Licenses and Certifications

  • Valid Pharmacist License in the state of Idaho (or ability to obtain prior to start date).
  • Current Basic Life Support (BLS) Provider certification through one of the following:
    • American Safety and Health Institute
    • American Heart Association
    • American Red Cross

Knowledge, Skills, and Abilities

  • Strong knowledge of medication reconciliation, discharge planning, and transitions of care best practices.
  • Understanding of outpatient pharmacy workflows, insurance processes, and medication access barriers.
  • Excellent communication and patient education skills.
  • Ability to collaborate effectively with interdisciplinary healthcare teams.
  • Strong attention to detail and commitment to patient safety.
  • Proficiency with electronic health records (e.g., Epic) and pharmacy systems.

Work Environment

This position works in both inpatient and outpatient pharmacy settings, including bedside patient interactions. Responsibilities may involve standing for extended periods, navigating patient care units, and coordinating a cross multiple departments to facilitate safe and timely discharge.
