Position Summary

Under the direction of the Central Intake Supervisor, the Central Intake Coordinator is responsible for receiving and processing all referrals in a timely manner, providing excellent customer service, effectively communicating both internally and externally, and promoting collaboration among the multi-disciplinary home health team.

College Degree in Nursing - Preferred

Experience

1 Year nursing, paramedical, or coding - Preferred

1 year nursing experience in an acute care setting or ICD coding preferred.
- Familiarity with medical terminology preferred but not required.

License & Certification

If a Licensed RN/LPN, must be in the State served by the Agency.

Certified EMT, paramedical, or coder preferred.

Core Job Functions
- Receive and process all referrals in a timely manner, including insurance and eligibility verification, diagnosis coding, procuring all required supporting documentation and accurately documenting in the home health application(s).
- Effectively communicate with all referral sources, physicians, patients, and co-workers when processing referrals. Provide any follow-up communication required timely. Participate in the on-call duties.
- Track and monitor all pending referrals in an effective and timely manner.
Salary : $20 - $31
