Join MiraSol Health Inc as a Central Intake Coordinator . This vital role ensures all referrals for both hospice & palliative care services are processed timely from receipt through admissions. It involves managing patient intake processes with compassion and efficiency, contributing to our mission of providing exceptional community healthcare.
Responsibilities:
- Responsible for access to care including the assurance that admission to hospice care services and palliative care services is available 24/7/365
- All necessary data, clinical information and notations uploaded into the electronic medical record in real time
- Oversees patient referral process, ensuring all documentation is complete, and patient has access to care within 24 hours of referral except when otherwise specified by the patient or designee.
- Ensure patient care delivery meets all state and federal requirements via relevance to central intake and referral process.
- Assists with hospice / palliative admission when all other resources are maximized.
Job Type: Full-time
Pay: $60,000.00 - $70,000.00 per year
