Responsibilities:- Coordinate and manage patient care plans, ensuring each individual receives personalized, compassionate attention.
- Conduct regular home visits to assess patient conditions and provide necessary medical interventions.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care strategies.
- Educate patients and their families about disease processes, treatment plans, and end-of-life care options.
- Maintain accurate and timely documentation of patient interactions and care plans in compliance with regulatory standards.
- Advocate for patient needs and preferences, ensuring their voices are heard and respected in all care decisions.
- Participate in team meetings to discuss patient progress and adjust care plans as needed to optimize outcomes.
- Experience in hospice or palliative care, demonstrating a deep understanding of patient-centered approaches.
- Ability to coordinate and manage comprehensive care plans, ensuring patient comfort and dignity.
- Proven track record of collaborating effectively with interdisciplinary teams to deliver holistic care.
