The Hospice RN Case Manager must be able to travel to our service areas of Morris County.
JOB RESPONSIBILITIES:
Provide skilled nursing care to hospice patients in accordance with their care plans.Assess patient conditions, manage symptoms, and adjust care plans as needed.
Educate and support patients, families, and caregivers on end-of-life care and comfort measures.
Coordinate with physicians, social workers, chaplains, and other team members to ensure holistic patient care.
Document patient status, treatments, and interventions in compliance with hospice regulations.
Participate in interdisciplinary team meetings and contribute to care planning.
Maintain compliance with Medicare, Medicaid, and state/federal hospice regulations.
Participate in on-call rotations as needed.
QUALIFICATIONS:
Education: Associate or bachelor’s degree in nursing (ASN/BSN) from an accredited program.Licensure: Active and unrestricted Registered Nurse (RN) license in New Jersey.
Experience: Minimum of two years of nursing experience, preferably in hospice, palliative care, or home health.
Certifications: CHPN (Certified Hospice and Palliative Nurse) preferred but not required.
Skills: Strong clinical assessment, communication, and organizational skills.
Must have a valid driver’s license, auto liability insurance, and reliable transportation. You must be willing to travel throughout the service area.
