What You'll Do
As a Care Manager, you'll play a key role in supporting clients and coordinating their care.
- Coordinate care and support treatment planning
- Provide counseling and rehabilitation services
- Conduct assessments and update care plans
- Help clients navigate healthcare and community resources
- Collaborate with providers, agencies, and care teams
- Maintain accurate and timely documentation
- Support clients with real-life challenges and health goals
What You Bring
We're looking for compassionate, driven individuals who want to grow in behavioral health.
You qualify if you meet one of the following:
- High school diploma 2 years of behavioral health experience
- Bachelor's degree 1 year of behavioral health experience
- Bachelor's degree in a human services field
- Completed Behavioral Health Technician (BHT) training program
- Master's degree in a human services field (not licensed)
Preferred:
- 1 year of behavioral health experience

Requirements
- Must be 18
- Valid driver's license with clean record
