JOB ANNOUNCEMENT

Title: Animal Care Coordinator / Case Manager  Division: HOPICS

FLSA: Non-Exempt, Full-Time  Supervisor: Program Manager

Pay Range or Rate: $26.00 - $28.00 per hour


Summary

Under the supervision of the Program Supervisor and Program Manager for Pathway Home, the Animal Care Coordinator / Case Manager provides support to individuals experiencing homelessness and their pets throughout SPA 6. This position is responsible for coordinating pet-related services, connecting participants to housing and community resources, maintaining pet care programs, and supporting housing stabilization efforts while ensuring the health, safety, and well-being of both participants and their companion animals.

Essential Functions

• Distribute pet-related supplies including food, crates, kennels, leashes, collars, and other necessary items to program participants.

• Maintain inventory and tracking systems for pet supplies and resources.

• Provide referrals and linkages to veterinary services, including vaccinations, spay/neuter services, microchipping, behavioral support, and general wellness care.

• Enroll participant pets into the PAS Program and maintain accurate records.

• Facilitate weekly meetings and engagement activities with pet-owning participants.

• Track pet-related expenditures and maintain required documentation in HMIS and internal tracking systems.

• Develop and maintain relationships with pet-friendly landlords to support participants in securing permanent housing.

• Promote a safe, supportive, and pet-friendly environment within Pathway Home Interim Housing.

• Educate participants regarding responsible pet ownership, animal care, safety, and wellness.

• Monitor and encourage daily care of participant pets, including access to food and basic necessities.

• Participate in staff meetings, trainings, and program development activities.

• Screen participants utilizing CES assessment tools, including VI-SPDAT, VI-F-SPDAT, and Next Step Tool.

• Maintain working knowledge of SPA 6 Coordinated Entry System resources and community services.

• Assist with outreach efforts, special projects, and community engagement activities as assigned.

• Prepare weekly and monthly reports related to participant and pet services.

• Assist with meeting contractual performance goals and reporting requirements.

• Participate in SPA 6 community meetings and collaborative efforts.

• Maintain accurate client records in compliance with HIPAA, 42 CFR Part 2, and all applicable funding requirements.

• Follow all SSG/HOPICS safety protocols, policies, and procedures.

• Maintain professional boundaries and adhere to SSG's Code of Ethics and HOPICS Core Values.

• Represent the Agency professionally at meetings, community events, and partner engagements.

• Regular attendance required.

Secondary Functions

• Assist E6 MDT Teams and Street Outreach staff with street-based engagement activities.

• Support HOPICS access points with participant triage, intake, enrollment, and service coordination.

• Assist with distribution of hygiene supplies, food, clothing, and pet-related resources to participants.

Minimum Qualifications - Knowledge, Skills and Abilities Required

• Bachelor's Degree preferred, or one to two (1-2) years of experience working in homeless services or a related field.

• Experience working with individuals experiencing homelessness and their companion animals preferred.

• Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds.

• Strong verbal and written communication skills.

• Ability to organize, coordinate, and implement program activities and services.

• Basic computer proficiency, including word processing and data entry systems.

• Ability to understand and carry out oral and written instructions.

• Valid California Driver's License, reliable transportation, and current automobile insurance required.

• Verification of Employment Eligibility and successful Background Clearance required.

• TB clearance required prior to hire or within seven (7) days of employment and renewed annually thereafter.

• CPR and First Aid certification required within 30 days of employment.

• If in recovery, a minimum of three (3) years drug and alcohol free is required.

• Experience conducting outreach and working with homeless individuals, families, community agencies, and pets preferred.

Supervisory Responsibilities

This position does not have supervisory responsibilities.

Environmental Conditions (Working Conditions)

This position is office and field based and requires travel throughout SPA 6. The incumbent must be comfortable working in fast-paced environments, shelters, community settings, and locations where clients may be experiencing crisis situations. The position requires flexibility, adaptability, and strong problem-solving skills.

Physical Requirements

The incumbent regularly sits, stands, walks, drives, speaks, listens, and carries materials weighing up to 20 pounds. Duties may include transporting supplies, assisting participants with personal belongings, attending meetings, and conducting outreach activities in the field.

Mental Requirements

This position requires the ability to manage changing priorities, interruptions, multiple deadlines, and high-volume workloads. The incumbent must be able to process information, exercise sound judgment, communicate effectively, work independently, and maintain professionalism while interacting with participants, community partners, and stakeholders.
