JOB ANNOUNCEMENT

Title: Resident Monitor  Division: HOPICS/CES for Pathway

FLSA: Non-Exempt, Full-Time  Supervisor: Pathway Home Program Supervisor/Manager

Pay Range or Rate: $23.20 - $24.72 per hour


Summary

Under the direction of the Program Supervisor/Manager, the Resident Monitor is responsible for client care, service coordination, facility safety, and supporting housing stabilization efforts. This position provides direct support to participants by assisting with service coordination, referrals, housing-related resources, and maintaining a safe and welcoming environment for all residents.

Essential Functions

• Provide light case management services, including developing individual housing service plans and connecting participants to needed resources.

• Meet regularly with participants to discuss needs, progress, goals, and barriers to housing stability.

• Coordinate with project staff and community partners to facilitate housing placement and supportive services.

• Provide participants with referrals and post-discharge information for wraparound services.

• Assist participants with employment readiness, training opportunities, and job placement resources.

• Coordinate ancillary services to promote housing stabilization and self-sufficiency.

• Maintain accurate and up-to-date documentation related to service coordination, housing placement, rental assistance, and move-in support.

• Collaborate with HOPICS staff, including housekeeping, maintenance, intern housing teams, and LAHSA.

• Provide diversion services as needed.

• Assist supervisors and managers with housing assessments and service coordination activities.

• Work collaboratively with outside agencies, community partners, and service providers.

• Enter all participant encounters and required information into HMIS within 24 hours.

• Prepare reports in accordance with funding requirements and Division policies.

• Assist in training and supporting staff, providers, and project partners participating in the program.

• Conduct criminal background checks on pre-eligible applicants and household members over the age of 13.

• Maintain client files and records in compliance with HIPAA, 42 CFR Part 2, and other funding requirements.

• Maintain professional boundaries and adhere to SSG’s Code of Ethics and HOPICS Core Values.

• Represent the Agency in a professional manner at meetings and community events.

• Regular attendance required.

• Perform other duties as assigned.

Secondary Functions

• Answer phones and route incoming calls as needed.

• Serve as back-up to the Intake Coordinator during absences.

• Perform additional duties assigned by the Senior Manager for Housing Services.

Minimum Qualifications - Knowledge, Skills and Abilities Required

• Associate Degree in Social Services, Social Work, Business, Customer Service, Case Management, or a related field from an accredited institution; OR equivalent combination of education and experience.

• Minimum one (1) year of experience working with individuals experiencing homelessness, mental illness, chronic health conditions, or other vulnerable populations.

• Working knowledge of Microsoft Office Suite, HMIS, and other database systems.

• Knowledge of community resources, case management practices, documentation standards, and housing services.

• Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds.

• Strong verbal and written communication skills.

• Verification of Employment Eligibility and successful Background Clearance required.

• TB clearance required prior to hire or within seven (7) days of employment and renewed annually thereafter.

• CPR and First Aid certification required within 30 days of employment.

• Valid California Driver’s License, current auto insurance, and reliable transportation required.

• Bilingual Spanish speaking, reading, and writing preferred.

• Experience working with homeless families, children, DCFS-involved families, and permanent housing placement preferred.

Non-Essential Qualifications

• Experience using HMIS preferred.

• HPRP experience preferred.

Supervisory Responsibilities

This position does not have supervisory responsibilities.

Environmental Conditions (Working Conditions)

This position works in client-friendly residential and shelter environments and may be required to visit shelters, homeless access centers, and other program locations. Local travel is required. The incumbent may work in environments where children and adults are present and where noise levels may vary.

Physical Requirements

The incumbent regularly sits, stands, walks, drives, types, carries materials up to 25 pounds, speaks, listens, and interacts with clients, staff, and community partners.

Mental Requirements

This position requires the ability to manage frequent interruptions, changing priorities, multiple deadlines, and a fast-paced environment while maintaining professionalism and quality service delivery.
