Director of Social Worker

South Shore Rehabilitation and Nursing, a skilled nursing facility located in Nassau County, New York is urgently seeking a full time Director of Social Worker (LCSW) to work at its facility. The ideal candidate will be a professional, compassionate and outgoing individual with work experience in the social services field, as well as experience in the skilled nursing/long-term care setting. The Director of Social Worker will plan, develop, organize, implement, evaluate, and direct social service programs in accordance with current existing federal, state, and local standards (as well as established facility policies and procedures), to assure that the medically related emotional and social needs of the residents are met and maintained on an individual client basis.

Requirements:

• Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in directing the overall operation of the facility's delivery of services.
• Long term care/ sub-acute experience required.
• Performs resident evaluations as required by state regulations.
• Support state regulations for admission and retention of residents.
• Assists residents and families with the move in process.
• Orients residents and families to the facility.
• Assists and interacts with residents and families to access services.
• Knowledge of discharge planning and assistance that address the identified needs of adult populations.

• Participates in development and implementation of each resident's individual service plan.
• Communicates with other members of the management team to ensure coordination of services to residents.
• Documents case management notes in residents' files as necessary.
• Oversees and coordinates the Individualized Service Plan (ISP) and the ALR personal data and resident evaluation form.
• May perform other duties as assigned.
• Knowledge of MDS and discharge plans are essential.

Qualifications:

• MSW, or Master’s Degree in a related field required.
• Experience in the field of human services or social work providing services to an adult population as required by state regulation if applicable.
• Two to five years experience in human services or social work providing services to an adult population is preferred.
• Basic computer knowledge, particularly in Microsoft Word.
• Certified/Licensed as required by state regulations if applicable.
• Valid driver's license.
