Position Overview:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide nursing support and direct care to individuals receiving services at our Serravezza Residential Home. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, focusing on person-centered care, medication management, stabilization, and coordination of services for adults with mental health diagnoses. Experience working in behavioral health is preferred. Must be available to work nights, weekends, and holidays.
Qualifications:
- High school diploma or equivalent required
- Successful completion of an LPN program with current licensure through the Alabama Board of Nursing
- Relevant nursing experience required; behavioral health or residential care experience preferred
- Strong communication, assessment, and clinical decision-making skills
- Ability to work effectively in a residential behavioral health setting
- Must be available to work nights, weekends, and holidays
- Must successfully pass a background check
Responsibilities:
- Provide nursing care and support for individuals receiving services in the residential home
- Conduct nursing assessments, monitor consumer health status, and document clinical findings
- Administer medications and treatments as directed by physician orders
- Provide education, training, testing, monitoring, evaluation, and updates for all MAC/MAS staff and nursing personnel
- Support stabilization efforts and continuity of care within the CCBHC care model
- Collaborate with multidisciplinary staff to promote recovery-focused, person-centered care
- Help maintain a safe, therapeutic, and structured living environment for residents
Work Environment:
- Work directly with adults receiving behavioral health services in a residential home setting
- Support daily wellness, medication management, and continuity of care for individuals with mental health diagnoses
- Engage in meaningful work that improves quality of life and promotes long-term recovery
