Position Overview:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide nursing support and direct care to individuals receiving services at our Serravezza Residential Home. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, focusing on person-centered care, medication management, stabilization, wellness support, and coordination of services for adults with mental health diagnoses in a residential behavioral health setting. Experience working in behavioral health is preferred. Must be available to work nights, weekends, and holidays.
Qualifications:
- Must have a High School Diploma or equivalent
- Successful completion of an LPN program with current licensure through the Alabama Board of Nursing
- Relevant nursing experience required; behavioral health or residential care experience preferred
- Strong communication, assessment, and clinical decision-making skills
- Ability to work effectively in a residential behavioral health environment
- Must be available to work nights, weekends, and holidays
- Must successfully pass a background check
Responsibilities:
- Provide nursing care and support for individuals receiving services in the Serravezza Residential Home
- Conduct nursing assessments, monitor consumer health status, and document clinical findings
- Administer medications and treatments as directed by physician orders
- Provide education, training, testing, monitoring, evaluation, and updates for MAC/MAS staff and nursing personnel
- Support stabilization, wellness monitoring, and continuity of care within the CCBHC care model
- Collaborate with multidisciplinary staff to promote recovery-focused, person-centered care
- Help maintain a safe, structured, and therapeutic residential environment for consumers
- Communicate with treatment team members regarding changes in consumer condition, medication concerns, or clinical care needs
- Maintain accurate, timely, and confidential documentation in compliance with agency standards, regulatory requirements, and CCBHC guidelines
Work Environment:
- Work directly with individuals receiving behavioral health services in a residential care setting
- Support medication management, wellness monitoring, stabilization, and continuity of care for adults with mental health diagnoses
