Position Overview:
We are seeking a dedicated and compassionate Community Health Worker to join our team in Andalusia. This part-time position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, supporting individuals and families in navigating behavioral health, medical, and community-based services through a person-centered, integrated care approach. This role plays a vital part in improving access to care, supporting service coordination, and helping consumers successfully navigate the healthcare system to promote wellness, recovery, and continuity of care.
Qualifications:
- Must have a High School Diploma or GED
- Strong understanding of healthcare delivery systems and the importance of integrated, whole-person care
- Experience or familiarity with care coordination processes, including transitions between providers, services, and levels of care
- Strong communication, advocacy, and relationship-building skills
- Ability to work effectively with individuals, families, healthcare providers, and community partners
- Proficiency in health IT systems and electronic documentation platforms preferred
- Additional certification in case management, community health work, or a related field highly desirable
- Knowledge of behavioral health services, community resources, and person-centered care principles preferred
Responsibilities:
- Support coordination of care across behavioral health, primary care, social services, and community-based providers to ensure consumers receive comprehensive, continuous services
- Assist consumers and families in understanding and navigating the healthcare system, available services, benefits, and community resources
- Help facilitate smooth transitions between levels of care, providers, and support systems to promote continuity and reduce barriers to treatment
- Collaborate with multidisciplinary team members including primary care providers, behavioral health clinicians, care coordinators, and social service agencies to support individualized care planning within the CCBHC care model
- Facilitate communication between consumers, families, and providers to ensure services align with individual needs, preferences, and recovery goals
- Maintain accurate, timely, and confidential documentation of care coordination activities in compliance with agency standards, privacy regulations, and CCBHC requirements
- Stay informed on best practices in care coordination, integrated care, and community health support through ongoing training and professional development
- Advocate for consumers and families by helping reduce barriers to healthcare access, treatment engagement, and community support services
