Operations and Planning
The Community Police Alliance Coordinator (CPAC) provides case management services for individuals with unmet human services needs referred by participating municipal police departments. The CPAC provides information, case planning, and closed-loop referrals to community services including mental health and substance use treatment, domestic violence services, housing, food, childcare, healthcare.
Essential Functions
- Provides outreach to individuals referred by participating municipal police departments.
- Engages individuals to create a Personal Action Plan that identifies the individual’s needs, goals, and joint action steps to implement the plan.
- Maintains records and collects data regarding referrals and case management outcomes.
- Provide follow-up to ensure that the referring law enforcement agency is kept informed of actions and outcomes.
- Respond to calls for immediate services during assigned working hours.
- Maintains a presence at the assigned municipal police department and meets regularly with assigned liaisons and other officers to promote effective communication and collaboration with the department.
- Collects data and documents all case management actions and communications in the CPAC data system.
- Develops a monthly report for the assigned police department documenting referrals, active caseload, and other relevant information regarding CPAC activities.
- Attends job related workshops, trainings and conferences as requested and approved by the supervisor.
- Provides community outreach as assigned by the supervisor.
Position Requirements
- Master’s degree in social work, criminal justice, psychology, or related field; LSW or LAC preferred
- A minimum of 1 year prior experience providing case management services to individuals with human services needs
- Demonstrated knowledge and understanding of human services systems and law enforcement
- Valid NJ Driver’s License
- Bilingual (Spanish/English) preferred
Salary: $50,000-$66,000/year
