Part Time- Full Time Hospice Case Managers
Requirements:
Unrestricted UT RN License
Home Health Case Management Experience
Must work well with Community Partners and Stakeholders
Current CPR Certification
Reliable car and drivers license
Exceptional Clinical Skills in Hospice and End of Life Care
Timely Charting Submission
Exceptional communication, team oriented, efficient work style, and ability to complete timely documentation a must.
