Solstice Home Health & Hospice is a locally owned organization providing high-quality home health and hospice services throughout Salt Lake County and the greater Wasatch Front area. We are currently seeking a Part-Time RN to provide skilled nursing care across both home health and hospice services.
This position is ideal for a compassionate, experienced nurse seeking flexible hours while delivering meaningful, patient-centered care.
Position Overview
The Part-Time Home Health & Hospice Nurse provides skilled nursing care to patients in their homes and care facilities, supporting both restorative home health services and end-of-life hospice care. This role requires strong clinical judgment, adaptability, and a commitment to individualized care.
Responsibilities
- Provide skilled nursing visits for home health and hospice patients
- Perform comprehensive assessments and ongoing patient evaluations
- Deliver symptom management, medication management, and patient/family education
- Collaborate with physicians and interdisciplinary team members to develop and update plans of care
- Accurately complete clinical documentation in compliance with state and federal regulations
- Communicate effectively with patients, families, and care team members
- Participate in interdisciplinary team meetings as assigned
- Provide limited on-call or weekend coverage, as needed and scheduled
Qualifications
- Current Utah RN license (required)
- Home health and/or hospice experience preferred
- Strong assessment, organization, and time-management skills
- Excellent communication and documentation abilities
- Ability to work independently in the field
- Compassionate, patient-centered approach to care
- Valid driver’s license and reliable transportation
- Must have great communication skills
- Timely work submission/on time with visits and paperwork
- Care deeply about serving older adults
