Hospice Case Manager (Full-Time)

Solstice Hospice & Palliative Care is a locally owned hospice committed to providing compassionate, high-quality, patient-centered end-of-life care. We are known for our clinical excellence, responsiveness, and strong interdisciplinary team culture. We are currently seeking a Full-Time Hospice Case Manager to join our dedicated clinical team.
This role is ideal for a skilled and compassionate nurse who values autonomy, meaningful patient relationships, and delivering care with dignity and integrity.
Position Overview
The Hospice Case Manager is responsible for coordinating and providing comprehensive nursing care to hospice patients and their families. This role serves as the primary clinical lead for an assigned patient caseload, collaborating closely with the interdisciplinary team to ensure individualized, high-quality care throughout the hospice journey.
Responsibilities
- Manage and coordinate a caseload of hospice patients in home, facility, and assisted living settings
- Perform comprehensive nursing assessments and develop individualized plans of care
- Provide skilled nursing visits, symptom management, and patient/family education
- Coordinate care with physicians, social workers, chaplains, aides, and other team members
- Ensure accurate, timely clinical documentation in compliance with hospice regulations
- Participate in interdisciplinary team meetings and care planning
- Provide on-call support as assigned (rotational)
- Advocate for patients and families with professionalism, empathy, and clinical expertise
Qualifications
- Current Utah RN license (required)
- Hospice experience strongly preferred (RN is Moving so full caseload to take over)
- Strong clinical assessment and critical thinking skills
- Excellent communication and documentation skills
- Ability to work independently while collaborating effectively with a team
- Compassionate, patient-centered approach to end-of-life care
- Reliable transportation and valid driver’s license
