Position: CCSP Care Coordinator
Location: Gwinnett County and Dekalb County
MUST RESIDE IN GEORGIA
Overview:
The CCSP Care Coordinator supports elderly individuals and those with disabilities by helping them access services needed to live independently in their communities. This role involves assessing client needs, developing care plans, coordinating services, and ensuring quality care.
Key Responsibilities:
- Assess Clients: Conduct IN PERSON initial and ongoing evaluations to determine eligibility and care needs.
- Develop Care Plans: Create and manage personalized care plans focused on independence and well-being.
- Coordinate Services: Arrange and monitor personal care, health services, and community resources.
- Monitor Progress: Follow up regularly to assess outcomes and adjust care as needed.
- Advocate and Educate: Support clients and families by ensuring understanding of services and rights.
- Ensure Compliance: Maintain accurate documentation and comply with state/federal guidelines.
- Manage Crises: Respond to emergencies and coordinate with healthcare providers as needed.
- Team Collaboration: Work with an interdisciplinary team to provide comprehensive care.
- Stay Informed: Participate in training and stay updated on best practices and policies.
Qualifications:
- Education: Bachelor's in Social Work, Nursing, Psychology, or related field (or equivalent experience).
- Experience: 1–2 years in care coordination, case management, or working with elderly/disabled populations.
- Skills: Strong communication, organizational, and problem-solving skills; knowledge of community resources and CCSP regulations; ability to work independently and with a team.

- Valid driver's license and reliable transportation
- Willingness to travel for home visits
- Background check/drug screening as needed

Combination of office and fieldwork with flexible scheduling based on client needs.
