This is Tues - Sat 8AM - 4PM, temp until early April. The Case Manager supports clients in achieving greater independence and stability while living in the community. Working collaboratively within a multidisciplinary team, the Case Manager provides services in psychiatric rehabilitation, medication and symptom management, relapse prevention, harm reduction, independent living skills, wellness coaching, housing support, budgeting, resource linkage, and community integration. This is onsite at only one location, but there is driving involved. The Case Manager typically needs to drive 2 - 3 times per week and may use a company vehicle.Qualifications:o Associate degree with 6 years’ experience in mental health as a specialist in physical restoration, social adjustment, or vocational adjustment,o OR Bachelor’s degree with 4 years’ experience in those fields,o OR Master’s degree with 2 years’ related experience.Company DescriptionWe are a staffing and recruiting agency that specializes in social services.
