Job Title: Case ManagerFLSA Classification: ExemptDepartment: Hanna Ahead: Mental Health & Wellness ServicesReports to: Clinical Program ManagerUpdated: 12.01.25Job SummaryThe Case Manager plays a critical role in the coordinated specialty care (CSC) program in directly supporting individuals through navigating community-based services, resource linkage, and support in crisis management under the supervision of the clinical program manager. They will support conversations on family needs identification (FNI), set case management goals with individuals, and facilitate access to community supports and resources for individuals and support persons. Additionally, they will assist with transitions from inpatient or higher levels of care to CSC, including transition planning once their time with the current program has concluded. The Case Manager will maintain up-to-date knowledge of a broad range of resources such as benefits, financial assistance, and housing options to provide personalized support to individuals and their families. They will also act as equitable consultants to clinical teams, promoting service user choice and participation in care and program activities while collaborating with other team members to ensure high-quality care delivery. Through assertive case management, the Case Manager will meet with individuals in their homes, schools, workplaces, and communities, fostering active participation in care. Their work emphasizes cultural sensitivity, individualized person-centered care, and helping individuals develop skills, establish daily routines, and pursue personally meaningful goals to enhance recovery and overall well-being.Key Responsibilities- Assist individuals in navigating, linking with, and obtaining benefits, financial assistance, housing options, and other resources.- Support individuals in building life skills, daily habits, and achieving meaningful recovery goals.- Coordinate community resources and family supports with attention and consideration of cultural relevance and accessibility.- Support transition planning, including timely implementation of plans for individuals with complex needs.- Provide education and consultation to individuals, families, and medical personnel regarding case management and transition planning.- Provide input to service users treatment plans and provide accurate documentation of sessions through collaboration with physicians, social workers, nurses, and other team members.- Contribute to program planning, goal setting, systems development, and process improvement.- Prepare and maintain required records and documentation.- Attend weekly team meetings Mondays 1 pm -3pm.- Consult with clinical team members and provide input to enhance service user care.- May assist in the collection and reporting of data for utilization of services, quality improvement, compliance, and other areas as needed.- Where applicable, arrange transportation with NMT (non-medical transport through Medi-Cal) for service users who are in need of transportation to and from their clinical appointments.- Other duties as assigned.Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.- Ability to assess individuals needs and develop plans of care with the team.- Ability to coordinate access to appropriate care levels, providers, settings, and services.- Proficiency in providing age-appropriate needs assessments and interventions.- Knowledge of available treatments, services, and resources for individuals.- Strong interpersonal skills.- Understanding of federal and state compliance laws and regulations for healthcare.- Knowledge of case management models, transition planning, utilization review, and quality assurance practices.- Familiarity with the continuum of mental health care services.- Respect for individuals’ beliefs, traditions, and identities.Core Competencies- Maintain the highest level of confidentiality.- Knowledge of care coordination and service planning- Knowledge of California systems of care- Knowledge of clinical documentation standards- Knowledge of crisis intervention and risk managementExperience- Minimum: One (1) year of experience providing case management services.- Preferred: Three (3) years of progressively responsible and directly related work experience.- Preferred: Two (2) years of experience working with individuals experiencing psychosis- Preferred: Experience working with youth and familiesRequirements- Bachelor’s Degree from an accredited college or university, or equivalent experience.- Certified or ability to become certified in CPR, First Aid, and Community Water Safety- Valid California driver’s license with the ability to become insured under company policy- Complete Mandated Child Abuse Reporting, Ethical Standards, Crossing Boundaries of Employee ConductPhysical Demands- Lift and/or carry up to 20 lbs. (occasionally)- Clear background check, pre-employment drug screening, and TB test- Sit for prolonged periods with the ability to perform repetitive movements.Work EnvironmentTypical office and clinical environment. Salary : $72,800 - $77,250
