What are the responsibilities and job description for the ADHC Case Manager position at Social Service Staffing & Recruiting, Inc.?
Schedule is M - F 8AM - 3:30PM, mainly at the ADHC with very minimal home visits. This is temporary, full time. There is no end date at this time. Training/ shadowing will occur in Pleasant Hill.
The Social Worker provides social services in accordance with Title 22 mandates to assist participants and their families to solve medical and psychological problems that arise because of the participant’s disability. The Social Worker works as a member of the team providing health and social services to frail elders and adults with disabilities to create opportunities where people can learn, grow and age independently with dignity and community.
Qualifications:
* BA degree in social work & 2 years experience as a Social Worker in a health related field, with demonstrated knowledge of case management.
* Possess knowledge of community resources for the disabled and elderly.
* Must have clean DMV record, reliable personal transportation, and auto insurance.
* TB test
* Must clear Livescan
