Position Overview
We are seeking a compassionate, self-motivated Case Manager to join our team. This individual will play a key role in assisting veterans with enrolling in VA services and identifying additional needs such as transportation, food, housing, financial support, mental health services, and other community resources. The ideal candidate is a strong advocate for veterans, thrives with minimal supervision, and has a heart for service. Preference will be given to applicants who are veterans themselves.
Responsibilities
- Assist veterans in navigating and signing up for VA Homemaker/Home Health Aide (HHA) and related services.
- Assess veterans’ needs and connect them to resources such as transportation, food programs, housing assistance, financial counseling, and mental health services.
- Serve as a trusted advocate for veterans and their families, ensuring they feel supported and respected.
- Build and maintain strong relationships with local organizations, nonprofits, and agencies serving veterans.
- Maintain accurate documentation of veteran interactions and services provided.
- Work independently to manage caseloads while contributing to the overall mission of Simplicity Home Care.
Qualifications
- Bachelor’s degree in Social Work, Human Services, Counseling, or related field (preferred but not required).
- Experience working with veterans or within the veteran community strongly preferred.
- Knowledge of VA programs, benefits, and community resources a plus.
- Strong interpersonal and communication skills with the ability to build trust quickly.
- Highly motivated, dependable, and capable of working with minimal supervision.
- Preference for candidates who are U.S. military veterans.

Pay: $20.00 - $23.00 per hour
Expected hours: 10 – 20 per week
