Hospice Registered Nurse Case Manager

Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our amazing hospice team. In this role, you will be responsible for coordinating patient care, ensuring effective discharge planning, and providing support to patients and their families throughout the healthcare continuum. This position is vital in managing patient experience and enhancing the quality of care delivered.

Responsibilities

• Develop and implement individualized care plans based on comprehensive assessments of patients' needs.
• Coordinate discharge planning to ensure a smooth transition from hospital to home or other care settings.
• Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to optimize patient care.
• Conduct diagnostic evaluations to assess patient conditions and determine appropriate interventions.
• Provide education and support to patients and their families regarding treatment plans and resources available.
• Monitor patient progress and adjust care plans as necessary to achieve desired health outcomes.
• Advocate for patients' needs within the healthcare system while ensuring compliance with regulations and standards.
