What You'll Do
- Manage and coordinate patient care in the home setting
- Perform skilled nursing assessments and visits
- Develop individualized care plans
- Educate patients and families to promote independence and healing
- Collaborate with physicians, therapists, and clinical team members
- Advocate for your patients every step of the way
- Complete timely and accurate documentation
What We're Looking For
- Current RN license in Oregon (or ability to obtain)
