As an Intake Coordinator for Homeland HomeHealth, you would contribute to our community of caring by evaluating patients referred to the organization, verifying insurance coverage and planning/coordinating the referrals for home health services. Another critical component of this role is establishing and maintaining positive relationships with customers and referral sources by responding to customer requests and concerns.

We believe in a multiple-disciplined approach to help patients in their healing journey, which includes wound care, cardiac care, palliative care, fall prevention, rehabilitation services, intravenous therapy, and other care services.

  • This is a Full-time position, Monday through Friday, 8:00 am to 5:00 pm, strictly in-person and office-based.
  • This position requires a valid Medical Assistant Certification, a valid Pennsylvania driver's license, and reliable internet access.

Essential Duties and Responsibilities include the following.

  • Evaluates all patients referred for home health services.
  • Gathers all necessary written information that includes intake data, essential background information, hospital course, homebound status, skilled home health needs and plan of care.
  • Completes accurate and timely entry of patient referral information and schedules admission visit as directed by the assistant director.
  • Ensures accurate and complete face to face encounter documentation prior to initiation of care.
  • Identifies and verifies insurance coverage of home health care services
  • Collaborates with discharge planning personnel and case managers to facilitate safe discharges.
  • Maintains liaison relationship with hospital personnel, providing information and education on organization services, coverage issues and related areas.
  • Works collaboratively with the marketing staff, directing personnel to make on sight facility visits when needed.
  • Communicates with receiving facilities when patients are readmitted, providing hand off information and decreasing the potential for partial episode payments.
  • Participates in patient care conferences and inservices as appropriate.
  • Collects and maintains statistical data on all referrals and submits them regularly as required.
  • Attends meetings and participates in committees as required.
  • Maintains the confidentiality of patient and organization information at all times.
  • Supports office and field staff by communicating messages via phone and fax.
  • Participates in the maintenance of workplace safety.
  • Participates in performance improvement activities, as needed and assigned.
  • Performs other duties as assigned by the Director.

Requirements include:

  • Education and Experience Requirements:
    • High School graduate or equivalent.
    • Completion of approved Medical Assistant program with current certification.
    • Proficient computer skills in business applications.
    • Knowledge of office equipment (i.e. printer, fax, computer) use and simple maintenance.
    • Possess strong written, verbal, and presentation skills with diverse populations.
    • Ability to apply problem-solving techniques to assigned duties.
    • Ability to multi-task effectively.
    • Ability to collect, interpret, and/ or analyze complex data and information.
    • Minimum age requirement of 18.
    • Minimum of one (1) year of experience working in a health care setting with community health/ home care experience preferred.
    • Ability to work within an interdisciplinary setting.
  • Licensure, Registration, and Certification Requirements:
    • Valid Pennsylvania Licensed Practical Nurse license.
    • Current certification as a Medical Assistant.
    • Satisfactory references from employers and/or professional peers.
    • Satisfactory criminal background check.
    • Maintains current CPR certification.
    • Access to reliable internet access outside of the work place setting.
