As a CNA for Homeland HomeHealth, you would contribute to our community of caring by providing direct support to patients where they feel most comfortable, promoting and fostering their healing process.

Homeland HomeHealth is a fast-growing provider of in-home healthcare that offers convenient and effective skilled nursing care in the comfort of our patient’s own home. Using compassion and skill, our nurses provide physician-ordered medical treatment to promote healing, independence, and the best possible quality of life. We believe in a multiple-disciplined approach to help patients in their healing journey, which includes wound care, cardiac care, palliative care, fall prevention, rehabilitation services, intravenous therapy, and other care services.

• This position requires a valid Pennsylvania CNA Certification, a valid Pennsylvania driver’s license, and reliable internet access.

As a HomeHealth CNA you would provide direct personal care to patients in their place of residence according to an established individualized plan of care and under the supervision of a Registered Nurse (RN) to promote patient comfort and dignity.  You would observe the patient, report and document observations and care performed. You will also be required to provide timely documentation of services based on the patient’s care plan. You will also provide assistance with basic office tasks as your schedule permits.

CNA Requirements include:

• Experience and Education

• • Minimum age requirement of 18.
  • Minimum of one (1) year of direct patient care experience in an institutional or home care setting required, with community health/ home care experience preferred.
  • High school diploma or equivalent.
  • Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements under CMS § 483.151 through § 483.154 and is currently listed in good standing on the State nurse aide registry.
  • Must demonstrate skills and have a passing score on a written competency test in accordance with 42 CFR Section 484.36 prior to working independently with a patient.
  • Must maintain compliance with CMS § 484.36 (b)(2) by attending at least 12 hours of ongoing educational in-service instructions annually  as provided by Homeland HomeHealth.

• Licensure, Registration, and Certification Requirements:
  • Valid Pennsylvania CNA Certification.
  • Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania state requirements and agency policy.
  • Satisfactory references from employers and/or professional peers.
  • Satisfactory criminal background check.
  • Maintains current CPR certification.
  • Access to reliable internet access outside of the workplace setting.
