Responsibilities:
- Perform comprehensive patient evaluations and develop individualized treatment plans based on physician orders, patient goals, and clinical findings
- Provide therapeutic interventions, rehabilitation techniques, and mobility training within the scope of practice established by state law and professional standards
- Assess patients’ functional status and continuously monitor progress toward established rehabilitation goals
- Educate patients, families, and caregivers on treatment plans, therapeutic exercises, mobility techniques, and use of adaptive or assistive devices
- Collaborate with interdisciplinary healthcare teams to coordinate patient care and support optimal clinical outcomes throughout the continuum of care
- Prioritize caseloads and coordinate daily patient scheduling, staffing needs, and care transitions as appropriate
- Maintain accurate and timely clinical documentation in accordance with organizational, regulatory, and payer requirements
- Support a positive patient experience through compassionate care, effective communication, and patient engagement initiatives
Qualifications:
- Degree
Doctor of Physical Therapy (DPT) degree preferred
- License(s) or Certification(s)
Basic Cardiac Life Support (BLS) certification required within 30 days of employment start date
- Job Experience and/or Industry Experience
