The Physical Therapist will collaborate closely with physicians, nursing teams, case management, rehabilitation staff, and caregivers to develop and implement personalized plans of care that support patient recovery across the continuum of care. Responsibilities:

• Perform comprehensive patient evaluations and develop individualized treatment plans based on physician orders, patient goals, and clinical findings
• Provide therapeutic interventions, rehabilitation techniques, and mobility training within the scope of practice established by state law and professional standards
• Assess patients’ functional status and continuously monitor progress toward established rehabilitation goals
• Educate patients, families, and caregivers on treatment plans, therapeutic exercises, mobility techniques, and use of adaptive or assistive devices
• Collaborate with interdisciplinary healthcare teams to coordinate patient care and support optimal clinical outcomes throughout the continuum of care
• Prioritize caseloads and coordinate daily patient scheduling, staffing needs, and care transitions as appropriate
• Maintain accurate and timely clinical documentation in accordance with organizational, regulatory, and payer requirements
• Support a positive patient experience through compassionate care, effective communication, and patient engagement initiatives

Qualifications:

• Degree

Bachelor’s degree in Physical Therapy required
Doctor of Physical Therapy (DPT) degree preferred

• License(s) or Certification(s)

Current State Licensed Physical Therapist required or eligible for licensure within the state of practice
Basic Cardiac Life Support (BLS) certification required within 30 days of employment start date

• Job Experience and/or Industry Experience

Previous experience in acute care, rehabilitation, outpatient therapy, home health, or hospital-based settings preferred but not required

Experience working with interdisciplinary healthcare teams and diverse patient populations preferred

• Software Skills

Experience utilizing Electronic Medical Records (EMR/EHR), rehabilitation documentation systems, and Microsoft Office Suite preferred

• Other Skills

Ability to deliver compassionate, patient-centered care in a fast-paced healthcare environment while maintaining high clinical standards
