Position Highlights
- Full-time opportunity
- Monday–Friday, 7:30 AM–4:30 PM
- Medical, dental, vision, and 401(k) benefits
- PTO and sick leave
- U.S. Citizenship
- Bachelor of Science in Nursing (BSN) from an ACEN- or CCNE-accredited program
- Current, unrestricted RN license from any U.S. jurisdiction
- 2 years of full-time Disease Management and Care Coordination experience within the last 3–4 years
- Experience managing chronic conditions such as diabetes, hypertension, COPD, or heart failure
- Current BLS certification
- Strong patient education and care coordination skills
- Develop and monitor chronic disease management plans
- Educate patients on disease prevention and self-management
- Coordinate care with providers and multidisciplinary teams
- Track patient progress and support improved health outcomes
