Position Highlights
- Full-time opportunity
- Monday-Friday (40hrs per wk)
- Medical, dental, vision, and 401(k) benefits
- PTO and sick leave
- U.S. Citizenship
- Associate Degree in Nursing or higher from an ACEN- or CCNE-accredited program
- Current, unrestricted RN license from any U.S. jurisdiction
- 3 years of RN Case Management experience within the last 4 years
- Case Management certification (CCM, CDMS, CRRN, or other PWS-approved credential)
- Current BLS certification
- Coordinate inpatient treatment and discharge planning
- Assess patient needs and develop individualized care plans
- Collaborate with physicians, nurses, and social workers
- Facilitate transitions to outpatient and community services
