Social Services Assistant

Job Summary
Shorehaven is seeking a compassionate and organized Social Services Assistant to join our team. In this role, you will support residents and families by assisting with admissions, resident transitions, documentation, and day-to-day social services operations.
The Social Services Assistant plays an important role in helping residents feel welcomed, supported, and connected throughout their stay. This position works closely with residents, families, and the interdisciplinary care team to promote quality of life and person-centered care.

Responsibilities
- Support the Social Services Department with daily resident and family needs
- Assist with new resident admissions, orientation, and paperwork completion
- Prepare admission packets, forms, notices, and related documentation
- Communicate with residents, families, and responsible parties regarding social service needs
- Maintain resident files, social service records, and department tracking logs
- Assist with MDS interviews and related documentation as assigned
- Help coordinate meetings related to resident care, admissions, rehab, or discharge planning
- Deliver required notices and resident communications
- Assist with resident transition planning and follow-up as needed
- Support department projects, education initiatives, and training programs
- Collaborate with nursing, therapy, and other interdisciplinary team members to support resident well-being
Qualifications
- High school diploma or equivalent required
- Prior office, administrative, healthcare, or social services experience preferred
- Long-term care or senior living experience preferred
- Strong communication and customer service skills
- Strong organizational skills and attention to detail
- Ability to manage multiple tasks and changing priorities
- Ability to maintain confidentiality and professionalism in sensitive situations
- Knowledge of HIPAA and resident rights preferred
Physical Requirements
- Ability to sit, stand, and walk throughout the workday
- Occasional bending, reaching, and lifting up to 20 pounds
- Ability to complete written and electronic documentation accurately
