What are the responsibilities and job description for the Discharge Care Navigator (Full Time, Towson Campus) position at Sheppard Pratt?
Responsibilities:

- In collaboration with the patients' interdisciplinary treatment team, performs a full range of aftercare and discharge planning services geared to mentally ill patient population.
- Maintains comprehensive knowledge base of agencies, organizations, and resources within the referral community for discharge planning and placement.
- Communicates with patients based on an understanding of the developmental needs of the population.
- Develops and assists in the implementation of discharge and aftercare plans; works with patients and families to facilitate the community transition and assure appropriate match to patients' needs.
- Bachelor's degree in a related mental health care field, such as Social Work, Nursing, Psychology, or a Rehabilitation Services specialty.
- For internal candidates, high school education plus an additional 5 years of relevant experience (2 years of work must be within SPHS) may be considered a substitute for a bachelor’s degree.
- Knowledge of psychiatric terminology, discharge and aftercare planning methods, and community referral resources - acquired through six to twelve months of relevant field or work experience in a psychiatric setting.
