Key Responsibilities of the Home Health Physical Therapist
- Evaluate patients in their homes and develop individualized treatment plans
- Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, and functional independence
- Educate patients and their families on exercises, mobility techniques, and injury prevention
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
- Maintain accurate clinical documentation and complete all required reports in a timely manner
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care
- Ensure compliance with home health regulations, safety standards, and infection prevention protocols

Requirements of the Home Health Physical Therapist Role
- Current BLS certification
- Current West Virginia Physical Therapist license
- Minimum 2 years of recent Home Health Physical Therapist experience
- Strong assessment, clinical reasoning, and rehabilitation skills
- Excellent communication, documentation, and patient care abilities
