Serenity Hospice is expanding into Western Pennsylvania and we’re looking for a compassionate and dedicated Hospice Social Worker to join our growing Monroeville team. This is an exciting opportunity to help build a new territory while providing meaningful support to patients and families during life’s most important moments.
What You’ll Do
Provide emotional, psychosocial, and practical support to hospice patients and their families
Conduct psychosocial assessments and contribute to individualized care plans
Assist families with community resources, advance care planning, and care transitions
Provide grief and bereavement support and education
Collaborate with nurses, chaplains, aides, and leadership as part of the interdisciplinary team
Help establish Serenity Hospice’s presence in the Western PA community

Qualifications
BSW or MSW from an accredited social work program
Healthcare experience required; hospice experience strongly preferred
Strong communication, assessment, and relationship-building skills
Ability to work independently and as part of an interdisciplinary team
Valid driver’s license and reliable transportation for field visits
