Responsibilities
Provide emotional support and counseling to hospice patients and their families
Assist patients and families with coping strategies, grief support, and end-of-life planning
Coordinate community resources and services for patients and caregivers
Collaborate with the interdisciplinary hospice team to ensure comprehensive patient care
Maintain accurate documentation in accordance with hospice regulations
Qualifications
Master’s Degree in Social Work (MSW) preferred
Current Pennsylvania Social Work license preferred
Hospice or healthcare experience is a plus
Strong communication and compassion for patients and families
