Registered Nurse (RN) – Hospice Case Manager
Location: Lehigh Valley, PA (ZIP Code 19518 and surrounding areas)
Schedule: Full-Time | No Nights, No Weekends
Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our interdisciplinary hospice team serving the Lehigh Valley area. This role focuses on providing high-quality, patient-centered care to individuals and their families during end-of-life, while supporting comfort, dignity, and quality of life.

Key Responsibilities
Manage and coordinate care for an assigned hospice caseload
Perform comprehensive patient assessments and develop individualized plans of care
Provide skilled nursing visits in patients’ homes and other care settings
Collaborate with physicians, social workers, chaplains, aides, and other team members
Educate patients and families on disease progression, symptom management, and medications
Monitor patient status and adjust care plans as needed
Ensure accurate, timely clinical documentation in accordance with hospice regulations
Participate in interdisciplinary team meetings and care planning
Serve as a patient and family advocate throughout the hospice journey
Qualifications
Current Pennsylvania RN license (required)
Minimum 1 year of nursing experience (hospice, home health, or case management preferred)
Strong clinical, organizational, and communication skills
Compassionate, patient-focused approach to care
Valid driver’s license with reliable transportation
