Overview
Serenity Estates of Freeport is seeking a compassionate and skilled Physical Therapist to join our dedicated in-house therapy team on a PRN basis. In this role, you will provide high-quality, patient-centered care aimed at improving residents' mobility, functional independence, and overall quality of life.
Key Responsibilities
- Evaluation & Assessment: Conduct comprehensive initial and ongoing evaluations to assess residents' physical abilities, functional status, and rehabilitation needs.
- Treatment Planning: Develop and implement individualized, evidence-based treatment plans tailored to the goals of each resident.
- Therapeutic Intervention: Administer various techniques and exercises designed to improve movement, manage pain, and restore function.
- Progress Monitoring: Track and document residents’ progress, adjusting treatment plans as necessary to achieve optimal clinical outcomes.
- Documentation: Maintain accurate, timely, and compliant records per federal, state, and facility standards.
Qualifications
- Licensure: Must possess a valid and current Physical Therapist (PT) license in the state of Illinois.
