Job Overview
We are seeking a dedicated and compassionate RN to join our home health team. The ideal candidate will possess a strong clinical background and be committed to providing high-quality patient care. As a Registered Nurse, you will play a vital role in assessing patient needs, developing care plans, and implementing treatment protocols to ensure optimal health outcomes.
Service Area: Fresno, Clovis, Madera, Sanger, Selma, Kerman, Reedley, Fowler, Coarsegold, Friant, Tollhouse, and More. We have a 50 Mile radius from our Office in Clovis, CA.
Duties
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illnesses. Ability to read documentation on patient status as well as any orders that the patient's PCP may have provided.
- Must complete one weekend a month for on-call, answering phone calls and completing the SOCs on that weekend as well. If unable to do a full weekend then commit to either a Saturday or Sunday.
- Must have own reliable transportation to get you to the client's home
- Initiates the plan of care and makes necessary revisions as patient status and needs change; uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications, treatments, IV ABX, etc. as prescribed by the physician.
- Provide specialized care such as Wound vac teaching, ostomy care and variety of wound care
- Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Strong knowledge of medical records management and documentation practices.
- Minimum of 2 years of experience as a Registered Nurse with a valid license required
- Valid drivers license and automobile insurance as required by state law
- Reliable transportation and Comfortable driving to patient homes for patient visits
- Prior experience in Home Health, Hospice, or Home Care preferred
- Strong communication skills with the ability to educate patients effectively.
- Must be able to complete charting in a timely manner
