Role Description – Home Health Registered Nurse (RN)Granite Ridge Home Health Care is seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our growing team. This role is ideal for an independent, organized nurse who thrives in a flexible environment and enjoys building one-on-one relationships with patients.
Key Responsibilities:- Perform comprehensive patient assessments in the home setting
- Develop, implement, and update individualized care plans
- Administer medications and treatments as prescribed
- Provide wound care, IV therapy, catheter care, and other skilled nursing services
- Monitor and document patient progress accurately and timely
- Educate patients and families on disease management and self-care techniques
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Ensure compliance with all agency policies, state, and federal regulations

QualificationsRequired:- Active Registered Nurse (RN) license in good standing
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience (home health experience preferred)
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver’s license and reliable transportation
- Current CPR certification
- Experience with electronic medical records (EMR) systems- Kinnser Prefered
- Knowledge of Medicare/Medicaid home health regulations
- OASIS documentation experience
