- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to determine individualized care needs.
- Administer medications, injections, and other treatments following strict protocols and safety standards.
- Manage complex wound care, including dressing changes and maintaining sterile techniques for catheterization and other invasive procedures.
- Utilize electronic health record (EHR) systems Axxess to document patient care accurately and efficiently.
- Educate patients and their families on disease management, medication administration, nutrition, and self-care strategies to promote optimal health outcomes.
- Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to specialists such as physical therapists or social workers.
- Respond promptly to urgent situations with skills in triage, airway management, acute pain control, and emergency response protocols.
- Valid LVN license in good standing with current state certification.
- Proven experience in home health nursing or related settings such as long-term care facilities, assisted living communities, or hospitals.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and medication administration procedures.
- Familiarity with EMR/EHR systems like Epic or Axxess; experience with medical documentation is highly preferred.
- Demonstrated ability to perform patient assessments accurately and deliver compassionate care tailored to individual needs.
- Must be willing to drive to the patient's home in your own vehicle.

- Excellent communication skills for effective patient education and collaboration with healthcare teams.
- Ability to work independently in patients’ homes while adhering to safety standards such as HIPAA and infection control protocols.
