Join our dedicated healthcare team as a Licensed Vocational Nurse (LVN) in the Home Health setting! In this vital role, you will provide compassionate, high-quality nursing care directly in patients’ homes, promoting health, independence, and well-being. Your expertise will help deliver personalized care plans, monitor patient progress, and support families through their healthcare journeys. • Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to determine individualized care needs.
• Administer medications, injections, and other treatments following strict protocols and safety standards.
• Manage complex wound care, including dressing changes and maintaining sterile techniques for catheterization and other invasive procedures.
• Utilize electronic health record (EHR) systems Axxess to document patient care accurately and efficiently.
• Educate patients and their families on disease management, medication administration, nutrition, and self-care strategies to promote optimal health outcomes.
• Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to specialists such as physical therapists or social workers.
• Respond promptly to urgent situations with skills in triage, airway management, acute pain control, and emergency response protocols.

• Valid LVN license in good standing with current state certification.
• Proven experience in home health nursing or related settings such as long-term care facilities, assisted living communities, or hospitals.
• Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and medication administration procedures.
• Familiarity with EMR/EHR systems like Epic or Axxess; experience with medical documentation is highly preferred.
• Demonstrated ability to perform patient assessments accurately and deliver compassionate care tailored to individual needs.
• Must be willing to drive to the patient's home in your own vehicle.
• We have a 50-mile radius from the office. We service Fresno, Fowler, Madera, Kerman, Reedley, Coarsegold, Auberry and more
• Excellent communication skills for effective patient education and collaboration with healthcare teams.
• Ability to work independently in patients’ homes while adhering to safety standards such as HIPAA and infection control protocols. Pay: $45.00 - $55.00 per hour
