Position Status: Full Time - Day Shift
Position Location: Chesapeake, VA
Hourly Range: $28.16/hr - $46/hr

Coordinates and directs the referral activities from health care facilities to home care by identifying/determining appropriateness of care, gathering medical and financial data, meeting with patients and families, and working with care management, physicians and referral sources to ensure a smooth transition. Responsible for safe and effective transfer from facility to home, and securing patient referrals by maintaining customer relationships.

- Need identification and coordination of services to meet patient need
- Physician order obtainment
- Patient and family management
- High School Diploma (Required)
- Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI
- Valid license in state of practice required:

- 1 year of home care, case management or discharge planning experience (Required).
