Overview:
Sentara Health is seeking to hire qualified individuals to join our team as Care Transition Coordinators.
Position Status: Full Time - Day Shift
Position Location: Hampton Roads area in Virginia
Hourly Range: $28.16/hr - $46.94/hr

Overview:
Care Transition Coordinator (CTC) serves as the liaison between inpatient providers, discharge planners, and our home-based services. This high-visibility role combines clinical knowledge with strategic sales acumen, playing a critical role in identifying appropriate patients, educating care teams, and influencing discharge decisions to support optimal post-acute transitions. CTC builds and maintains strong relationships with referral sources and educates patients/families on available services. Collaborates with operational teams to ensure timely admissions and optimal patient experience.
The ideal candidate is a results-oriented professional who understands the nuances of referral development, is comfortable navigating acute and post-acute settings, and thrives in a relationship-driven environment.

High School Diploma (Required)
Certification/Licensure:
RN, LPN, BSW/MSW, LPTA, RT (Required)

Experience:
- Minimum 1 year of experience in home health, hospice, hospital discharge planning, or care coordination (Required)
- Proven ability to influence clinical decision-makers and drive referrals
- Strong communication and interpersonal skills
