Position Overview
We are seeking a compassionate and skilled Physical Therapist (PT) with lymphedema treatment experience to join our home health team serving patients throughout the Treasure Coast. This role focuses on delivering high-quality, individualized therapy services in patients’ homes while promoting functional independence, safety, and overall well-being. The ideal candidate is clinically confident, self-motivated, and comfortable working autonomously in a field-based environment.
Key Responsibilities

Evaluate and develop individualized treatment plans for home health patients.

Provide skilled physical therapy interventions in accordance with physician orders and care plans.

Perform lymphedema evaluations and treatment, including manual lymphatic drainage, compression therapy, bandaging, and patient education.

Monitor patient progress and modify treatment plans as needed.

Educate patients and caregivers on exercises, mobility techniques, fall prevention, and condition management.

Complete timely and accurate documentation in EMR system.

Communicate effectively with physicians, nurses, case managers, and interdisciplinary team members.

Ensure compliance with Medicare, state, and agency guidelines.
Qualifications

Current Florida Physical Therapist license in good standing.

Certified Lymphedema Therapist (CLT) or documented clinical experience treating lymphedema required.

Minimum 1 year of PT experience (home health preferred).

Current BLS/CPR certification.

Valid driver’s license and reliable transportation.

Strong clinical judgment and documentation skills.

Ability to work independently and manage a flexible field schedule.

Home health or geriatric experience.

Familiarity with OASIS documentation
