Overview:
We are seeking a compassionate and skilled Speech Therapist (ST) to provide high-quality, patient-centered speech therapy services on a PRN/Part Time basis in the home health setting. In this role, you will work one-on-one with patients to address communication, swallowing, and cognitive-linguistic needs—helping them regain function and improve their quality of life from the comfort of their homes.
This role offers flexible scheduling, independence in the field, and the support of a collaborative clinical team.
Key Responsibilities:

Conduct comprehensive speech, language, cognitive-linguistic, and swallowing evaluations.

Develop individualized treatment plans based on evaluation findings and patient goals.

Provide skilled therapy interventions, including communication therapy, swallowing therapy, cognitive retraining, and compensatory strategy training.

Educate patients and caregivers on home exercises, safety, and communication strategies.

Document all care in compliance with Medicare and agency standards.

Communicate regularly with nurses, therapists, and other interdisciplinary team members to ensure coordinated care.

Report any significant changes in patient condition to the physician and clinical team.

Active Florida Speech Therapist License.

Minimum of one (1) year of home health experience.

Strong clinical assessment and communication skills.

Reliable transportation, valid driver’s license, and current auto insurance.

Ability to work independently and manage a PRN caseload.
