Position Overview
Senior Care Solutions is seeking a compassionate and motivated Physical Therapist (PT) to join our growing Medicare-certified home health team. This Baylor PRN position is ideal for clinicians looking to work weekend shifts while maintaining flexibility during the week.
As a Physical Therapist with Senior Care Solutions, you will provide skilled therapy services to patients in their homes, helping them regain mobility, improve strength, and enhance their overall quality of life.
Key Responsibilities

Perform comprehensive physical therapy evaluations, including SOC, Recertification, and Discharge assessments

Develop and implement individualized treatment plans based on patient needs and goals

Provide therapeutic exercises, gait training, balance training, pain management, and functional mobility interventions

Educate patients and caregivers on home exercise programs, safety, and fall prevention

Accurately complete all documentation in the EMR within required timeframes

Collaborate with physicians, nurses, therapists, and interdisciplinary team members

Participate in case conferences and care coordination as needed

Maintain compliance with Medicare, ACHC, and agency standards
Qualifications

Licensed Physical Therapist in the state of Florida (active and in good standing)

Minimum of one (1) year of physical therapy experience

Home health experience strongly preferred

Strong clinical assessment and documentation skills

Ability to work independently and manage a field-based schedule

Reliable transportation, valid driver’s license, and active auto insurance

Current CPR certification
