PRN Occupational Therapist (OT) – Home Health
Location: Deerfield Beach, Hillsboro Beach, Pompano Beach, Lauderdale-By-The-Sea, Fort Lauderdale, Dania Beach, and Hallandale Beach.
Position Summary:
We are seeking a compassionate and self-motivated Occupational Therapist (OT) to join our Home Health team on a per diem (PRN) basis. In this role, you will provide individualized, in-home therapy services to help patients regain independence in their daily living and self-care activities. The ideal candidate enjoys working autonomously, thrives in a patient-centered environment, and is committed to improving quality of life for those we serve.
Responsibilities:

Perform initial and ongoing assessments of patients’ functional abilities, ADLs, mobility, safety, and home environment.

Develop and implement individualized care plans based on physician orders and patient needs.

Deliver therapeutic interventions including training in self-care, use of adaptive equipment, and home safety modifications.

Educate patients and caregivers to support recovery and independence.

Accurately document all care provided using WellSky.

Coordinate care and communicate effectively with the interdisciplinary home health team.

Maintain compliance with agency, state, and federal guidelines.

Current FL licensure as an Occupational Therapist (OT).

Minimum 1 year of clinical experience; home health experience preferred.

Familiarity with WellSky EMR or similar documentation systems preferred.

Strong clinical reasoning, time management, and communication skills.

Ability to work independently and manage a flexible field-based schedule.

Reliable transportation, valid driver’s license, and current auto insurance.
