Overview
We’re seeking an experienced and relationship-driven Clinical Liaison (LPN) to join our Medicare-certified Home Health Agency. This role plays a key part in connecting patients to skilled home health services through strong referral relationships and clinical expertise. The ideal candidate is an LPN who thrives in a fast-paced environment, enjoys building partnerships, and understands the importance of timely, compliant transitions of care.
Key Responsibilities

Develop and maintain referral relationships with hospitals, skilled nursing facilities, physician offices, and community partners to drive Medicare home health admissions.

Serve as a liaison between referral sources, patients, families, and our clinical team to ensure smooth coordination of care and timely start of services.

Conduct clinical assessments to confirm eligibility for Medicare home health services and assist in the coordination of admissions.

Educate healthcare providers and case managers on our agency’s services, admission criteria, and clinical programs.

Work collaboratively with intake, scheduling, and field clinicians to ensure seamless onboarding and communication.

Monitor referral trends, report outcomes, and support agency growth initiatives.

Represent the agency professionally at community events, health fairs, and educational presentations.

Active LPN license in the state of Florida (required).

Minimum of 2 years of experience in home health, hospice, or hospital discharge planning (Medicare-certified agency experience preferred).

Knowledge of Medicare regulations, OASIS requirements, and home health eligibility criteria.

Strong interpersonal, communication, and presentation skills.

Proven ability to establish and maintain productive relationships with referral partners.

Reliable transportation, valid driver’s license, and auto insurance.
